Tara Suzette Jervis Boyd, 33, of Hazard, died Friday July 27, 2012 at her residence. She was born July 5, 1978 to the late Debra Lou Jervis Henley. She was the fiancee of Joshua Campbell.
She is survived by her grandmother: Mary Fulford of Prestonsburg; sister: Nicky Adams of Columbus, OH; special niece: Montana Adams; and a host of aunts, uncles, family and friends.
Funeral services ill be held Thursday August 2; 12:00 noon at Faith Independent Freewill Baptist Church in Prestonsburg, with Donnie Patrick officiating.
Burial will follow the service at Little Mary Church Cemetery in Louisa, KY.
Nelson Frazier Funeral Home in charge of arrangements.
